When the program is open, you will find an Apply Now button on this page. When the program is closed, we have a form where you can add your email for notification of when the program is open again.
Can I purchase a handful of boxes for our clinic to hand out to families?
Due to grant-tracking the applications must be filled out by the families with the families’ own information, including mailing address; however, clinics, social workers, aides, etc may assist with applying. If you are assisting with families applying, please leave a comment in the comment section specifying.
Can you send us a sample box so that we can show families what they can apply for?
